These terms and conditions apply to all coaching services and communication, email or otherwise, delivered by Sarah Gold, (the Coach), as well as information set forth on this website (www.sarahgoldwellness.com).
The role of the Coach is not to prescribe or assess micro and macronutrient levels; provide health care, medical or nutrition therapy services, or to diagnose, treat or cure any disease, condition, or other physical or mental ailment. Rather, the Coach is a mentor and guide who has been trained as a Functional Medicine Certified Health Coach to help clients reach their own health goals by devising and implementing positive, sustainable lifestyle changes. Sarah Gold Wellness is not acting in the capacity of a medical doctor, licensed dietician, nutritionist, or personal trainer, and any advice given by the Coach is not meant to take the place of advice by these professionals. If you are under the care of a health care professional or currently use prescription medications, you should discuss any dietary changes or potential dietary supplement use with your primary care physician and should not discontinue any prescription medications without first consulting your primary care physician. Health coaching is not a substitute for medical attention, examination, diagnosis, or treatment.
By purchasing coaching services from Sarah Gold Wellness, you confirm that you have read and agree to each statement:

I understand that the coaching services I will be receiving from the Coach are not offered as a substitute for professional mental health care or medical care and are not intended to diagnose, treat or cure any mental health or medical conditions.

I understand and agree that I am fully responsible for my well-being during my coaching sessions, and subsequently, including my choices and decisions.

I understand that coaching results are not guaranteed, and no promises have been set forth regarding results of the coaching engagement.

I hereby release, waive, acquit and forever discharge my Coach, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by the Coach resulting from the coaching relationship contemplated by this agreement. This agreement shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
